"Is PCREE testing the same as calibration?" is one of the more common questions from administrators new to equipment compliance — understandably, since both are performed by biomedical technicians, both result in a certificate, and both show up in the same equipment maintenance conversation. They measure fundamentally different things, though, and a facility that only does one is missing part of its compliance obligation.
The Core Difference
PCREE testing checks whether a device is electrically safe. Calibration checks whether a device's measurements are accurate. A patient scale can be perfectly safe electrically — no leakage current risk, solid ground path — while reading 8 pounds off due to calibration drift. Conversely, a scale can be perfectly calibrated and accurate while having a frayed power cord that creates a genuine electrical safety hazard. Neither test substitutes for the other.
What PCREE Testing Actually Checks
PCREE (Patient Care Related Electrical Equipment) testing, required under NFPA 99 Chapter 10, measures chassis and patient leakage current, ground resistance, and polarity using a calibrated electrical safety analyzer. It answers the question: if something goes electrically wrong inside this device, is a resident or staff member protected from harm? It applies to any electrically powered device with patient care contact — beds, lifts, monitors, pumps, and scales among them.
What Calibration Actually Checks
Calibration measures whether a device's output — a weight reading, a blood pressure value, a temperature — matches a known reference standard within an acceptable tolerance. A scale is calibrated against NIST-traceable reference weights; a vital signs monitor is calibrated against a reference signal generator. Calibration doesn't touch electrical safety at all — a device can be recalibrated to perfect accuracy while its power cord ground pin is corroded and its leakage current is out of spec.
Why Most Facilities Need Both
Any electrically powered measurement device in a skilled nursing facility — patient scales being the clearest example — typically needs both PCREE electrical safety testing and calibration, on their own separate schedules, with separate documentation. CMS surveyors reviewing equipment records may ask about either, and a facility that can produce a calibration certificate but not a PCREE test record (or vice versa) has only addressed half of what a complete equipment compliance program covers for that device.
Devices that typically need only PCREE testing, not calibration: hospital beds, patient lifts, and other equipment that doesn't produce a measured clinical value. Devices that typically need both: patient scales, vital signs monitors, and any other device where both electrical safety and measurement accuracy matter.
Scheduling Both in the Same Visit
Most facilities find it efficient to schedule PCREE testing and calibration for the same devices during the same technician visit — the technician is already on-site and set up at the device, and consolidating both into one visit reduces the number of separate service calls a facility needs to coordinate throughout the year.
