Why Infusion Pumps Are High-Stakes PCREE Equipment
Beyond the general electrical safety concern shared by all PCREE devices, infusion pumps have direct patient contact through the IV line and are often in continuous operation for extended periods, sometimes running overnight unattended. A chassis leakage fault or ground failure on a device in continuous contact with a resident's vascular system is treated by CMS and NFPA 99 as a materially more serious risk category than, say, a scale used for a few minutes during a weekly weigh-in.
What Gets Tested
Standard PCREE electrical safety testing applies: chassis and patient leakage current, ground resistance, and polarity, using a calibrated electrical safety analyzer, performed by a qualified technician (in practice, a CBET). Given the continuous, unattended operation common with infusion pumps, technicians performing PCREE testing on this equipment typically also visually inspect the power cord, battery backup function (many infusion pumps run on battery during patient transport), and door/latch mechanisms for physical integrity as part of the same service visit — not strictly a PCREE electrical test requirement, but standard practice given the device's risk profile.
Testing Frequency and Post-Repair Requirements
Annual testing is the NFPA 99 minimum, but the pre-use and post-repair testing requirements carry particular weight for infusion pumps given how frequently they cycle through service for battery replacement, door mechanism repair, or software updates. Every infusion pump returning from any repair — not just an electrical repair — should be re-tested before being placed back into resident use. A facility that tests infusion pumps annually but skips post-repair retesting has a documentation and compliance gap that surveyors are specifically trained to probe given the equipment category's risk profile.
Rental and Leased Pump Fleets
Many SNFs supplement their owned infusion pump inventory with rental or leased units, particularly for short-term or high-acuity residents. PCREE testing responsibility for rental equipment should be addressed explicitly in the rental agreement and the facility's Equipment Management Plan — either the rental company certifies current testing before delivery (with documentation the facility retains), or the facility tests the unit itself upon receipt. What a surveyor won't accept is silence on the question: rental status doesn't exempt equipment from PCREE requirements while it's in use in a certified facility.
Documentation Surveyors Expect
Each infusion pump — owned, rented, or leased — should appear in the equipment inventory with a current test record: measured leakage current and ground resistance values, test date, and the technician's CBET credential number. For rental units, the facility should retain either the rental company's current test documentation or its own test record, whichever applies under the facility's policy.
